Medical care that meets you where you are.

Medica Home Calls provides primary and supportive care for medically complex adults—delivered by telehealth and, when available, through home visits. We focus on continuity, medication clarity, and careful planning across care settings.

Care model

This practice exists for people whose needs are not met by rushed, office-based medicine. We take responsibility for the whole picture: symptoms, medications, transitions, and the human realities that shape health.

What we do

  • Primary care for medically complex adults
  • Supportive care alongside serious or chronic illness
  • Medication reconciliation and simplification
  • Care coordination across specialists and settings
  • Goals-of-care and advance care planning

What we don’t do

  • Emergency response (call 911 for emergencies)
  • “Drop-in” urgent care or walk-in services
  • One-time opinions without follow-through

If you are unsure whether this fits, send an inquiry. We will help you choose the right next step.

Physician leadership

Medica Home Calls is led by Patrick Blanchard, M.D., a board-certified family physician with more than 30 years of clinical experience across rural practice, hospital care, hospice, and home-based settings.

The practice is built around a simple conviction: medically complex patients do better when care is slow enough to be accurate, coordinated, and humane.

Who we serve

Medica Home Calls is designed for patients and caregivers managing complexity—multiple diagnoses, multiple medications, multiple care settings, and too many rushed appointments.

Older adults

People who want steady, longitudinal care and find clinic visits burdensome, confusing, or unsafe.

Senior communities

Residents in assisted living, memory care, or skilled nursing who benefit from continuity and careful medication oversight.

Caregivers and families

Adult children, partners, and friends who need a steady clinical point of contact and a coherent plan.

Where care can happen

We meet people in the places that matter most. When a video visit is appropriate, we use a secure platform and keep the technology as simple as possible.

What guides the work

The point of home-based care is not convenience. It is accuracy, continuity, and dignity—care that fits the life the patient is actually living.

Clarity

We speak plainly about uncertainty and trade-offs so decisions can be made without pressure or jargon.

Continuity

We reduce fragmentation by staying involved through transitions—hospital, rehab, home, and facility care.

Respect

Serious illness changes lives. We build plans around goals, limits, and what matters to the patient.
